Benefit and Pension Plan Support Services

I. Enrolment and Ongoing Benefit Administration Support

Enrolling new client employees' to their respective benefit plans:

  • Process and submit application for group coverage form and trustee appointment form for those who choose a beneficiary that is under the ago of 18;
  • Client employee and client employer orientation to selected benefit plan coverage (i.e. Life Insurance, Accidental Death & Dismemberment, Short Term Disability, Long Term Disability, Health & Dental) and additional services included in the group (i.e. group net, best doctors, global medical assistance, employee assistance program);
  • Enroll clinet employee and distribute information booklets including pay direct drug cards, additional services brochures, expense forms i.e. health, dental, and vision care...;
  • Maintain up to date records for each client employees' benefits (Original forms, correspondences' with insurer etc.);
  • Register the trustee appoinment.

Adjusting Benefit Coverage

  • Moving from client employer coverage to personal coverage including, processing Group Life Conversion forms (to switch coverage to individual coverage);
  • Termination of coverage

Short Term Disability Management (If Applicable)

  • Process the employer's form for Short Term Disability;
  • Process the client employee's form and the attending Physician's Statement;
  • Submit paperwork to insurer and work with insurer claims adjusters to ensure accurate and timely processing of claims;
  • Liaise between insurer and the cleint employee to avoid delays in the processing claims.

Long Term Disability Management

(a) Processing

  • Process the employer's form for Longt Term Disability;
  • Process the client employee's form and the attending Physician's Statement;
  • Submit paperwork to insurer and work with insurer claims adjusters to ensure accurate and timely processing of claims;
  • Liaise between insurer and the cleint employee to avoid delays in the processing claims.

(b) Life Claim Waiver Management

  • Process the Client employer's Statement for the Life Claim Waiver form after the 119 day elimination period to waiver Life Insurance premium payments;
  • Process the client employee claim by ensuring completion of the employee's statement form and the attending physician's statement;
  • Submit paperwork (Life Cliam Waiver) to insurer and the client employerr;
  • Liaise between insurer claims adjusters and the client employee to ensure that Long Term Disability is processed in a timely and accurate manner.

Life Claim Report Management (if plan member or dependant becomes deceased)

  • Process Part 1 of the life claim report (employer's statement);
  • Work with client employee to ensure proper completion of client employee form;
  • Process the forms and submit to insurer
  • Liaise with insurer and client employee to ensure accurate and timely payment of the claim

Accidental Death and Dismemberment Management

  • Process Part 1 of the life claim report (employer's statement);
  • Work with client employee to ensure proper completion of client employee form and attending physician statement;
  • Collect and submit required additional forms including; official death certificate, police report, or client employer's accident report;
  • Process the claim and submit all paperwork to insurer
  • Liaise with claims adjusters to ensure the timely payment of the claim

Process Trustee Appointment Changes

  • Re-register as required beneficiaries of insurance policies

On Going Personal Support to Client and Client Employee

  • On-going, liaise with clinet employees' including answering all questions regarding benefits;
  • If client employee is declined we will advocate on behalf of the the client employee and we will negotiate with claims adusters to ensure all opportunity is given for successful processing of his/her claim.
